Finding the right place for your loved one is so important. At Sylvan Heights, you’ll find skilled caregivers who have successfully helped residents who are confused, have dementia, behavioral problems, Alzheimer’s, diabetes, Parkinson’s, incontinence, or are on hospice service. Many family members praise our caregivers for literally changing the lives of their loved ones. That’s because our residents feel safe and loved and are well cared for.

All care is under the direction of each resident’s personal physician, an experienced RN and a 24-hour, on-site CNA. We offer:

• Daily monitoring and distribution of medication (per MD’s orders)
• Three home-cooked meals a day plus nutritious snacks
• Private rooms with attached half bath
• Weekly/as-needed housekeeping services with laundered bed and bath linens
• Daily personal laundry

We customize a care plan for each resident, which can include:

• Personal hygiene assistance
• Incontinence care
• Foley catheter care
• Colostomy care
• Shower assistance
• Dressing assistance
• Mobility assistance or transfers
• Behavior management
• Diabetic care/insulin injections
• Oxygen therapy
• Complicated wound care
• Physical therapy
• Hospice care
• Nighttime care and assistance
